Diabetes and Endocrinology

Diabetes and Endocrinology

Our Diabetes and Endocrinology service provides education and treatment plans for individuals with diabetes (Healthy WA) and other endocrine conditions.

Our services include:

  • diabetes self-management and education for people with type 1 and type 2 diabetes
  • high risk podiatry care only (basic care can be assessed in the community)
  • insulin initiation and stabilisation for people with type 1 and type 2 diabetes
  • gestational diabetes/diabetes in pregnancy
  • general endocrinology problems.

Our multidisciplinary team includes:

  • endocrinologists
  • credentialled diabetes educators
  • dietitians
  • podiatrists
  • psychologists.

Working together we deliver a comprehensive clinical service to patients referred from:

  • within Rockingham General Hospital
  • other health care organisations
  • the local community.

We also work collaboratively with our patients’ GPs to support patients to self-manage their condition. This helps minimise the impact of their condition on their lifestyle and the risk of complications developing or progressing.

Referrals

A medical referral is required for an individual appointment with an endocrinologist.

Referrals are accepted from GPs for access to our credentialled diabetes educator, dietitian and podiatry services.

A referral from either an endocrinologist, general practitioner, nurse or other allied health care workers is required for diabetes education.

All referrals are managed through the Central Referral Service (external site) or phone 1300 551 142.
