What is the Home Hospital service?
The Silver Chain Home Hospital (external site) is a metropolitan-based service that provides healthcare services in your home.
It involves 4 complementary services.
- Priority Response Assessment (PRA) provides a 24 hour, 7 days a week nursing service that is supported by a network of GPs.
- Post Acute Care (PAC) services are nursing services provided immediately after an eligible patient has been discharged from a public hospital or another Home Hospital service.
- Hospital at the Home (HATH) is a hospital substitution program which is available 24 hours a day, 7 days a week. Access to this service generally follows an admission to a public hospital.
- Community Nursing (CN) provides an alternative to hospital for patients who need nursing at home and who are not eligible for Home and Community Care (HACC) nursing.
Who can use the service?
Home Hospital services are for any adult living in the community or in residential aged care who has acute or post acute care needs that can be treated effectively and safely in the person’s place of residence.
Cost
There is no charge for users of this service.
To apply
Referrals are normally through a GP, registered nurse, public hospital or associated hospital programs.
