WA Rheumatic Heart Disease Register access statement
The Western Australian Government collects and safeguards client/patient data in accordance with the Health (Rheumatic Heart Disease Register of Western Australia) Regulations 2015 and the Health Services Act 2016.
All staff who have been granted access to the WA Rheumatic Heart Disease (RHD) Register are reminded of their responsibility to maintain patient confidentiality according to the WA Department of Health Code of Conduct.
Who can have access to the WA RHD Register?
- WA RHD Control Program Staff
- Healthcare professionals involved in the direct care or care coordination of clients with a record on the WA RHD Register
To ensure privacy of information on the WA RHD Register, all users are required to:
- Not disclose your password and/or username to anyone
- Only use information for direct care or coordination of care of clients
- Store and transmit information in a secure manner
- Advise the Register by email RHD.Register@health.wa.gov.au if you see incorrect information
What is the process to gain access to the Register?
- Read this access statement thoroughly
- If you agree with the terms set out in this document, complete the user access application form (page 2 of this document) and email the form to the Register
- You will be advised of the outcome of the application
- If successful, the Register Data Entry Officer will contact you to provide login details and arrange training
Access Limitations
- Users on short-term contracts will be provided with access until their contract end
- Users with permanent contracts will be provided with ongoing access
- Accounts which are inactive for more than 6 months will be deleted
- Users must inform the WA RHD Register and Control Program when access is not needed
- Users are only provided with read-only access
Use of the data for education, research or quality improvement purpose
- Please do not extract data direct from the Register for these purposes
- If information is sought for use in presentations, publication or research, please submit a data request to RHD.Register@health.wa.gov.au.Requests can take 2-4 weeks to respond to
