Models of care

Health Networks led the development of 58 Models of Care and 9 Frameworks which provide outlines for gold standard practice in a variety of health areas. Hundreds of people were involved in the development of the Models of Care and Frameworks including clinical staff, health service planners, non-government organisations, general practitioners, advocates, consumers, and carers.

A report on the implementation (PDF 1.3MB) of the models of care and frameworks shows there has been significant progress made.
