Rockingham Peel Group Aged Care Transition and Liaison Nurse Deb Jones received top honours at the recent WA Nursing and Midwifery Excellence Awards,

Deb was named WA Nurse/Midwife of the Year after also taking out the award for the Excellence in Primary, Public and Community Care category

Deb was nominated for her deep commitment to supporting Rockingham General Hospital (RGH) patients to achieve their health goals after leaving hospital

Deb has helped hundreds of elderly patients transition from hospital discharge to home through the creation of individualised treatment and management plans and conducting follow-up home visits

“I really enjoy working with my patients to help them achieve their health goals and improve their quality of life,” said Deb.

“I am proud of being able to provide patient centred care, including chronic disease education and working on management plans with the patient and all members of the primary health setting.”

RGH Aged Care Rehabilitation Nurse Unit Manager Joanne Hughes described Deb as a dedicated and committed nurse.

“Deb displays an outstanding level of understanding of how older patients are impacted by health care services,” said Joanne.

“She works exceptionally hard to reconnect patients with primary health care providers, arranging medical appointments, troubleshooting travel and collecting medications.

“Deb is well respected and valued within the community and her patients are always complimentary of her.”

Two other South Metropolitan Health Service staff won awards at the event which was postponed last year due to the COVID-19 pandemic.

Sharon Rowe, Adult Burns Service Clinical Nurse Consultant at Fiona Stanley Fremantle Hospitals Group (FSFHG), won the Excellence in Rural & Remote Health award for supporting and educating nursing staff in remote and rural areas to provide high quality burns care.
